3.) 🚩ATTACH Your Horse’s Documents:
•A USEF Horse Vaccination Record dated within 6 months of the show date.
•A Negative Coggins dated within one year of the show date.
The horse's name + Year of Birth on your entry form must be the same as on the Coggins report and USEF / USDF documentation.

4.) Stabling Request/Grounds Fee
Include the dates/add the proper amount.
** If you are NOT STABLING, you must select the Grounds Fee with entry.**

5.) Any Additional Info:
-- • A copy of Qualifying Test for Musical Freestyle
-- • IF YOU ARE ENTERING A TEST OF CHOICE, BE SURE TO PUT THE TEST YOU WISH TO RIDE IN THE "Test Of Choice" BOX
-- • Your Horse's TIP # on entry form if you wish to participate in the Thoroughbred Incentive Program
-- • IF YOU ARE ENTERING A MINOR, PLEASE put the parent/guardian’s contact information and USEF/USDF membership information in the Parent/Guardian box

🔹 Fees automatically added to entry 🔹
• $35.00 Processing Fee per entry.
• $23.00 USEF Federation Fee per entry. (Includes $15 for Drugs and Medication Fee and $8 Federation fee)

🔹 Incomplete entries or entries with non-matching paperwork may be charged an incomplete entry fee, which must be paid for ride times to be assigned. 🔹
